A consortium agreement is a contract between the student, Ohio University, and the Host (Partner) Institution. Because you are enrolled in the Online Bachelor Completion Program, Ohio University will grant your degree upon completion of the degree requirements. This agreement permits the Ohio University Office of Student Financial Aid and Scholarships (OSFAS) to process financial aid based on the combined registered hours at both colleges for each term of the academic year.

YOU MUST COMPLETE THIS DOCUMENT AND FOLLOW INSTRUCTIONS FOR SUBMISSION IF YOU WILL BE RECEIVING FINANCIAL AID FOR THE ONLINE BACHELOR'S DEGREE COMPLETION PROGRAM.
